To enable this Bureau to complete its records connected with your Regiment, and to

facilitate the operation of settling your accounts when your term of service shall expire, you
will please fill up the enclosed statement as accurately as possible, as indicated by the
printed form, and return the same to this Office without delay. The information required

can easily be obtained from the Company or Regimental Monthly Returns, or from the

retained Muster Rolls.
